TSP PressIndiana DOL References ANSI E1.212012-02-09 On February 8, the Indiana Department of Labor announced the citations it has issued related to the Indiana State Fair stage collapse, which occurred on 13 August 2011. Complete scans of the official "Safety Order and Notification of Penalty" documents issued against Mid-America Sound Corporation, IATSE Local 30, and the Indiana State Fair Commission are provided. Failure to follow the requirements of ANSI E1.21 is a major part of the findings. The efficacy of the consensus standard's inspection requirements is noted at about the 51-minute mark in the press conference audio recording.
